Registering with the Surgery
You may register by coming into the surgery and completing a registration form of by bringing in your signed NHS medical card. You have a right to express a preference for a certain health professional to the reception.

Information about you is held in the surgery on paper records and a CD. The computer records are accessed through the internet. Your records are the property of the state. The surgery is obliged to grant access to the state (the NHS and its agents). Please contact the Practice Manager if you have any concerns about your data.
Services Provided under the Contract:
3 yearly checks (yearly if over 75)
You may ring the surgery and make and appointment with the nurse.
New patient check
If you are new to the surgery you may book an appointment with the doctor or the nurse.
You may benefit from a check if you are using inhalers. For a review please book with the nurse.
The surgery offers an annual review.
All childhood jabs are available at the surgery. Please book with the nurse.
Child Health Surveillance
Child health checks are shared between the doctor and health visitor.
Please ask for an antenatal appointment if you are pregnant. You can book with the midwife (01795) 418300 x 3014.
All services except IUCD's (coils) and implants are available at the surgery. Follow up checks with the nurse. For a new or changed method see the doctor.
NHS England stopped commissioning Minor Surgery procedures from us in April 2015.
Dr Beerstecher is the named GP for patients.
